Our 2026 Needs Survey is an opportunity for you to help shape future service and support design in a rapidly evolving health and aged care landscape.

This 15 minute online survey is being conducted for WentWest by Outcome Services and will enable us to benchmark ourselves against other PHNs.
Your survey responses will remain anonymous unless you choose to have them linked to your name. If you enter the prize draw or request follow‑up, your contact details will be collected in a separate database and will not be linked to your survey responses. Providing contact details is optional; if you don’t provide them, you can still complete the survey but we won’t be able to contact you or enter you into the prize draw.
